RESIDENTIAL
LIVING LEVEL OF CARE IN AN ALF:                                $2,850

1.
Rated dependent in only one of seven ADLs; OR         

2. Rated dependent in one or more of four selected IADLs; OR

3. Rated dependent in medication administration.

REGULAR ASSISTED
LIVING LEVEL OF CARE IN AN ALF:  
                     $3,450

1.
Rated dependent in two or more of seven ADLs; OR

2. Rated dependent in behavior pattern. 
 
INTENSIVE ASSISTED LIVING LEVEL OF CARE IN AN ALF:                     $4,150
HOSPICE LEVEL OF CARE IN AN ALF “Intensive”                                
                                                                                                 
(only private pay qualifies for intensive)
:

1.
Rated dependent in four or more of seven ADLs; OR

2.
Rated dependent in two or more ADLs and rated as semi-dependent or dependent in a combination of behavior pattern and orientation; OR

3.
Rated semi-dependent in two or more ADLs and rated as dependent in a combination of behavior pattern and orientation.

4.
Patients are referred to hospice when life expectancy is approximately six months or less. Hospice care can continue longer than six months, if needed, but requires physician certification.
